Medication Pick-up by June 15 @ 12:30

We request that you pick up your child’s unused medication by 12:30 on June 15. If you cannot come to the school by that time, please make arrangements with the office staff to pick it up as soon as possible. If unused medication is not picked up on the last day of the current school year, it will be disposed of by school personnel. We are not allowed to carry medication over to the next year.

NEW Asthma Action Plans / Health Treatment Plan

Please remember that NEW Asthma Actions plans, Allergy Plans and/or Health Treatment Plans need to be filled out every year by your child’s Doctor’s office. These can be printed online from the Clinic Page or you may get one in the clinic before summer break begins.
